Alprazolam is a triazolo analog of the 1,4 benzodiazepine class of central nervous system-active compounds. Available as tablets in 0.25 mg, 0.5 mg, 1 mg, or 2 mg strengths.

Alprazolam is a triazolo analog of the 1,4 benzodiazepine class of central nervous system-active compounds. Available as tablets in 0.25 mg, 0.5 mg, 1 mg, or 2 mg strengths.

Uses

Indicated for acute treatment of generalized anxiety disorder (GAD) in adults and treatment of panic disorder with or without agoraphobia in adults.

Dosage

For GAD: start 0.25-0.5 mg three times daily, increase every 3-4 days to maximum 4 mg daily in divided doses. For Panic Disorder: start 0.5 mg three times daily, increase every 3-4 days by no more than 1 mg per day. When discontinuing, decrease dosage by no more than 0.5 mg every 3 days.

Side Effects

Most common adverse reactions include drowsiness, light-headedness, dizziness, and impaired coordination. Other reactions include dry mouth, hypotension, memory impairment, irritability, and dysarthria. Discontinuation-emergent symptoms include insomnia, anxiety, muscle twitching, and gastrointestinal effects.

Interactions

Strong CYP3A inhibitors (except ritonavir) are contraindicated. Opioids increase respiratory depression risk. CNS depressants produce additive effects. Ketoconazole increases alprazolam concentrations 3.98-fold. Carbamazepine may decrease alprazolam levels.

Warnings

Concomitant use with opioids may cause profound sedation, respiratory depression, coma, and death. Risk of abuse, misuse, and addiction which can lead to overdose or death. Abrupt discontinuation or rapid dosage reduction may precipitate acute withdrawal reactions that can be life-threatening.

Pregnancy

Neonates born to mothers using benzodiazepines late in pregnancy may experience sedation and withdrawal symptoms. Monitor neonates for signs of sedation, respiratory depression, hypotonia, and feeding problems.

Storage

Store at 20° to 25°C (68° to 77°F). Keep out of reach of children.
