Uses

For the relief of symptoms of depression. Endogenous depression is more likely to be alleviated than are other depressive states.

Dosage

Initial adult dosage typically ranges from 75-150 mg daily in divided doses or 50-100 mg at bedtime. Hospitalized patients may require 100-200 mg daily. Adolescent and elderly patients should receive lower doses (10 mg three times daily with 20 mg at bedtime).

Side Effects

Includes cardiovascular effects (arrhythmias, hypotension), CNS effects (seizures, hallucinations), anticholinergic effects (urinary retention, constipation), allergic reactions, hematologic effects, gastrointestinal disturbances, and endocrine effects.

Interactions

Interactions with topiramate, CYP2D6 inhibitors, MAOIs, anticholinergic agents, cimetidine, and CNS depressants.

Warnings

BOXED WARNING: Antidepressants increased the risk of suicidal thinking and behavior in children, adolescents and young adults. Risk of serotonin syndrome, bipolar disorder screening needed, cardiovascular precautions required. Contraindicated with MAO inhibitors and in patients with prior hypersensitivity.

Pregnancy

Use only if potential benefit to the mother justifies the potential risk to the fetus. Drug crosses placenta; animal studies showed teratogenic effects at high doses.

Storage

Store at 20 to 25 degrees C (68 to 77 degrees F). Protect from light. Dispense in tight, light-resistant container with child-resistant closure.
