Uses
Indicated for the treatment of partial-onset seizures in patients 1 month of age and older.
Dosage
The recommended starting dosage for adults is 50 mg twice daily (100 mg per day) with adjustments from 25 mg to 100 mg twice daily based on patient response. Pediatric dosing is weight-based. Hepatic impairment requires dose reduction.
Side Effects
Most common adverse reactions (at least 5%) are somnolence/sedation (16%), dizziness (12%), fatigue (9%), and nausea/vomiting.
Interactions
Rifampin decreases brivaracetam concentration (increase dose up to 100%). Increases carbamazepine metabolite levels. Increases phenytoin levels up to 20%. No added benefit when combined with levetiracetam.
Warnings
Antiepileptic drugs including brivaracetam increase the risk of suicidal thoughts or behavior. Warnings include neurological adverse reactions, psychiatric adverse reactions, hypersensitivity, serious dermatologic reactions (Stevens-Johnson syndrome), and withdrawal considerations. Contraindicated in hypersensitivity to brivaracetam.
Pregnancy
Available data from the NAAED pregnancy registry are insufficient to identify a risk of major birth defects. Animal studies showed developmental toxicity at higher exposures. Brivaracetam is present in breast milk.
Storage
Store at 20 to 25 degrees C (68 to 77 degrees F). Excursions permitted from 15 to 30 degrees C.
