Indicated for treating edema linked to congestive heart failure, hepatic and renal disease, including nephrotic syndrome. Effective via oral or parenteral routes.
Dosage
Standard initial dose is 0.5 to 1 mg intravenously or intramuscularly over 1 to 2 minutes. Additional doses may be given at 2 to 3 hour intervals, not exceeding 10 mg daily. Dosage should be individualized based on patient response.
Side Effects
Most frequent: muscle cramps (1.1%), dizziness (1.1%), hypotension (0.8%). Lab abnormalities include hyperuricemia, hypochloremia, and hypokalemia.
Interactions
Avoid with aminoglycoside antibiotics (ototoxicity risk), nephrotoxic drugs, lithium, probenecid, and indomethacin.

Bumetanide is a potent diuretic which, if given in excessive amounts, can lead to profound diuresis with water and electrolyte depletion. Contraindicated in anuria, hepatic coma, severe electrolyte depletion, and hypersensitivity to bumetanide. Risk of ototoxicity and sulfonamide cross-reactivity.
Pregnancy
No adequate controlled studies in pregnant women. Use only if potential benefit justifies fetal risk.
