Bupivacaine hydrochloride and epinephrine bitartrate injection is an amide local anesthetic with vasoconstrictor for the production of local or regional anesthesia or analgesia for surgery, dental procedures, diagnostic and therapeutic procedures, and obstetrical procedures.

Uses

Indicated in adults for the production of local or regional anesthesia or analgesia for surgery, dental and oral surgery procedures, diagnostic and therapeutic procedures, and for obstetrical procedures.

Dosage

Dosages vary based on anesthetic procedure, area to be anesthetized, tissue vascularity, and patient condition. Maximum daily dosage is 400 mg in 24 hours. For dental use, average dose is 1.8 mL per injection site. Epidural administration uses incremental doses of 3-5 mL.

Side Effects

Most common reactions affect the central nervous system and cardiovascular system including restlessness, anxiety, tremors, convulsions, cardiac arrhythmias, hypotension, and cardiac arrest. Allergic reactions and methemoglobinemia may also occur.

Interactions

Additive toxicity with other local anesthetics. Severe hypertension with MAOIs and tricyclic antidepressants. Interactions with ergot-type oxytocic drugs, nonselective beta-blockers, and potent inhalation anesthetics.

Warnings

Risk of cardiac arrest with difficult resuscitation or death during epidural anesthesia in obstetrical patients, particularly with 0.75% concentration. Contraindicated for obstetrical paracervical block, intravenous regional anesthesia (Bier Block), and hypersensitivity to bupivacaine or amide-type local anesthetics.

Pregnancy

Not recommended for obstetrical paracervical block. Animal studies showed embryo-fetal lethality. Local anesthetics cross the placenta. Bupivacaine is excreted in breast milk.

Storage

Store at 20-25 degrees C (68-77 degrees F). Solutions with epinephrine cannot be autoclaved and must be protected from light.
