Uses

Indicated for treating Parkinson's disease, post-encephalitic parkinsonism, and symptomatic parkinsonism following carbon monoxide or manganese intoxication.

Dosage

Initial dosage: one tablet of 25 mg/100 mg three times daily. Increase by one tablet daily or every other day as needed, up to eight tablets daily. Transfer from levodopa requires discontinuing it 12 or more hours prior.

Side Effects

Most common: dyskinesias and nausea. Others include cardiac irregularities, orthostatic hypotension, gastrointestinal bleeding, hallucinations, psychotic episodes, and impulse control symptoms.

Interactions

Antihypertensive drugs may require dosage adjustment. Selegiline may cause severe orthostatic hypotension. Dopamine antagonists reduce therapeutic effects. Iron salts reduce bioavailability.

Warnings

Carbidopa permits more levodopa to reach the brain, causing CNS effects like dyskinesias at lower doses. Risk of sudden sleep onset during activities. Monitor for depression with suicidal tendencies and neuroleptic malignant syndrome. Contraindicated with nonselective MAO inhibitors (discontinue 2 or more weeks prior), hypersensitivity to any component, and narrow-angle glaucoma.
