Uses

Treatment of lower respiratory tract infections, acute bacterial otitis media, skin and skin structure infections, urinary tract infections, uncomplicated gonorrhea, pelvic inflammatory disease, bacterial septicemia, bone and joint infections, intra-abdominal infections, meningitis, and surgical prophylaxis.

Dosage

Administered intravenously or intramuscularly. Adult dose: 1-2 grams daily. Pediatric doses vary by indication (50-100 mg/kg). IV infusion over 30 minutes recommended; neonates require 60 minutes.

Side Effects

Generally well tolerated. Common reactions include diarrhea (2.7%), rash (1.7%), eosinophilia (6%), thrombocytosis (5.1%), leukopenia (2.1%), and elevation of liver enzymes.

Interactions

Incompatible with vancomycin, amsacrine, aminoglycosides, and fluconazole in admixtures. Antagonistic effects observed with chloramphenicol. Probenecid does not alter elimination.

Warnings

Risk of hypersensitivity reactions in penicillin-sensitive patients. Interaction with calcium-containing products can cause precipitation. Neurological adverse reactions reported. Clostridium difficile-associated diarrhea possible. Hemolytic anemia risk. Contraindicated in premature neonates, hyperbilirubinemic neonates, and neonates requiring calcium-containing IV solutions.

Pregnancy

Animal studies at 20x human dose showed no embryotoxicity or teratogenicity. No adequate well-controlled human studies. Use only if clearly needed.

Storage

Store at 20-25 degrees C (68-77 degrees F), protected from light. Reconstituted solutions stable 2 days at room temperature or 10 days refrigerated.
