Uses
Indicated for osteoarthritis, rheumatoid arthritis, juvenile rheumatoid arthritis (age 2 and older), ankylosing spondylitis, acute pain, and primary dysmenorrhea.
Dosage
Osteoarthritis: 200 mg daily or 100 mg twice daily. Rheumatoid arthritis: 100-200 mg twice daily. Juvenile RA: 50 mg twice daily (10-25 kg) or 100 mg twice daily (over 25 kg). Ankylosing spondylitis: 200 mg daily, up to 400 mg if needed. Acute pain: 400 mg initially, then 200 mg as needed; 200 mg twice daily thereafter.
Side Effects
Most common (over 2%): abdominal pain, diarrhea, dyspepsia, flatulence, peripheral edema, dizziness, pharyngitis, rhinitis, sinusitis, upper respiratory infection, rash. Serious: cardiovascular thrombotic events, GI bleeding/ulceration/perforation, hepatotoxicity, anaphylaxis, serious skin reactions.
Interactions
Warfarin/anticoagulants increase bleeding risk. ACE inhibitors/ARBs/beta-blockers may have diminished effects. Diuretics' natriuretic effect reduced. Digoxin levels increased. Lithium clearance decreased. Methotrexate toxicity risk increased. CYP2C9 inhibitors/inducers affect celecoxib metabolism.
Warnings
NSAIDs cause increased risk of serious cardiovascular thrombotic events (MI, stroke) which can be fatal. Risk may occur early and increase with duration. Contraindicated post-CABG surgery. Increased risk of serious GI events (bleeding, ulceration, perforation). Contraindicated in hypersensitivity to celecoxib, history of asthma/urticaria/allergic reactions to aspirin/NSAIDs, and allergic-type reactions to sulfonamides.
Pregnancy
Avoid NSAIDs at approximately 30 weeks gestation or later due to premature ductus arteriosus closure risk. Limit use 20-30 weeks to lowest effective dose and shortest duration.
