Uses

Indicated for treatment of major depressive disorder (MDD) in adults. Citalopram is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder.

Dosage

Administer once daily with or without food. Initial dosage is 20 mg once daily; after one week may increase to maximum dosage of 40 mg once daily. Patients greater than 60 years, those with hepatic impairment, and CYP2C19 poor metabolizers: maximum recommended dosage is 20 mg once daily.

Side Effects

Most common adverse reaction (5% or more and twice placebo) is ejaculation disorder, primarily ejaculatory delay. Other common reactions include nausea (21%), dry mouth (20%), somnolence (18%), insomnia (15%), diarrhea (8%), tremor (8%), and increased sweating (11%).

Interactions

Significant interactions with MAOIs (contraindicated), pimozide (contraindicated), QT-prolonging drugs, CYP2C19 inhibitors (reduce to 20 mg daily), other serotonergic drugs, and drugs affecting hemostasis (antiplatelet/anticoagulant agents).

Warnings

WARNING: SUICIDAL THOUGHTS AND BEHAVIORS. Antidepressants increased risk of suicidal thoughts and behaviors in pediatric and young adult patients in short-term studies. Monitor all antidepressant-treated patients for clinical worsening and emergence of suicidal thoughts and behaviors. QT-Prolongation and Torsade de Pointes: Dose-dependent QTc prolongation and cardiac arrhythmias observed. Serotonin Syndrome: Risk increased with concomitant serotonergic agents. Contraindicated in patients taking MAOIs or within 14 days of stopping them, taking pimozide, or with known hypersensitivity to citalopram.

Pregnancy

SSRI use, particularly late in pregnancy, may increase risk for persistent pulmonary hypertension of newborn and poor neonatal adaptation symptoms. Exposure in month before delivery associated with less than 2-fold increase in postpartum hemorrhage risk.

Storage

Store at 20 to 25 degrees C (68 to 77 degrees F); excursions permitted to 15 to 30 degrees C (59 to 86 degrees F).
