Clopidogrel bisulfate is a thienopyridine class inhibitor of P2Y12 ADP platelet receptors. The tablets contain 97.875 mg clopidogrel bisulfate equivalent to 75 mg base.

Uses

Used to reduce myocardial infarction and stroke rates in patients with: acute coronary syndrome (non-ST and ST-elevation), recent MI, recent stroke, or established peripheral arterial disease. Clopidogrel tablets should be administered in conjunction with aspirin.

Dosage

For acute coronary syndrome: single 300 mg oral loading dose and then continue at 75 mg once daily. For recent MI/stroke/PAD: 75 mg once daily orally without a loading dose.

Side Effects

Bleeding, including life-threatening and fatal bleeding, is the most commonly reported adverse reaction. Additional serious events include thrombotic thrombocytopenic purpura, hypersensitivity reactions, and various dermatological reactions.

Interactions

CYP2C19 inhibitors reduce effectiveness; CYP2C19 inducers increase bleeding risk. Avoid concomitant use of omeprazole or esomeprazole. NSAIDs, warfarin, SSRIs/SNRIs increase bleeding risk.

Warnings

WARNING: DIMINISHED ANTIPLATELET EFFECT IN PATIENTS WITH TWO LOSS-OF-FUNCTION ALLELES OF THE CYP2C19 GENE. Tests identify poor metabolizers; consider alternative P2Y12 inhibitors for these patients. CYP2C19 inhibitor interactions (avoid omeprazole/esomeprazole), bleeding risk increase, premature discontinuation risks. Contraindicated in active pathological bleeding and hypersensitivity to clopidogrel.

Pregnancy

Available data from published literature and postmarketing surveillance with clopidogrel use in pregnant women have not identified any drug-associated risks for major birth defects. Animal studies showed no fetotoxicity at doses 65-78 times human dose.

Storage

Store at 20 to 25 degrees C (68 to 77 degrees F); excursions permitted to 15 to 30 degrees C. Protect from moisture.
