Dosage
Dosage must be adjusted to individual patient needs. The recommended intramuscular dose is 10 mg to 20 mg four times a day for adults, limited to 1-2 days when oral medication is unavailable.
Side Effects
Common side effects (greater than 5%) include dizziness, dry mouth, blurred vision, nausea, somnolence, asthenia, and nervousness. Clinical trials showed 61% of patients experienced anticholinergic effects.
Interactions
Antiglaucoma agents: anticholinergics antagonize effects and increase intraocular pressure. Other anticholinergics may increase side effects. Antacids interfere with absorption. Absorption of other drugs like digoxin may be affected.
Warnings
Inadvertent intravenous administration risks thrombosis; cardiovascular effects with tachyarrhythmia conditions; heat prostration with decreased sweating; psychosis/delirium in sensitive patients. Contraindicated in infants under 6 months, nursing mothers, unstable cardiovascular status, acute hemorrhage, myasthenia gravis, glaucoma, obstructive uropathy, GI obstruction, severe ulcerative colitis, and reflux esophagitis.
Pregnancy
This drug should be used during pregnancy only if clearly needed. Epidemiologic studies showed no increased malformation risk at doses up to 40 mg/day during first trimester.
