Varies by indication: 40 mg daily to 1.5 mg/kg daily subcutaneously. Adjusted dosing for renal impairment and geriatric patients 75 years and older.

Bleeding, anemia, thrombocytopenia, elevation of serum aminotransferase, diarrhea, nausea, ecchymosis.

Discontinue anticoagulants, antiplatelet agents, NSAIDs, and salicylates before use when possible.
