Esmolol hydrochloride is a beta-1 selective adrenergic receptor blocker with rapid onset and approximately 9 minute elimination half-life, administered intravenously.

Uses

Short-term IV treatment for supraventricular tachycardia rate control and perioperative tachycardia/hypertension management.

Dosage

IV infusion: 500 mcg/kg loading dose over 1 min, then 50-200 mcg/kg/min maintenance; titrate at 4-minute or greater intervals based on response.

Side Effects

Most common: symptomatic hypotension (12%), asymptomatic hypotension (12%), infusion site reactions (8%), nausea (7%).

Interactions

Digoxin increases bradycardia risk; anticholinesterases prolong neuromuscular blockade; sympathomimetics counteract effects; avoid vasoconstrictive agents.

Warnings

Risk of hypotension, bradycardia, cardiac failure; masks hypoglycemia symptoms; risk of unopposed alpha-agonism in pheochromocytoma. Contraindicated in severe sinus bradycardia, heart block greater than 1st degree, sick sinus syndrome, decompensated heart failure, cardiogenic shock.

Esmolol hydrochloride should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Unknown excretion in breast milk.
