Ezetimibe and simvastatin tablets contain ezetimibe (dietary cholesterol absorption inhibitor) and simvastatin (HMG-CoA reductase inhibitor). Available in strengths of 10/10, 10/20, 10/40, and 10/80 mg.
Uses
As an adjunct to diet to reduce elevated low density lipoprotein cholesterol (LDL-C) in adults with primary hyperlipidemia and pediatric patients aged 10 and older with heterozygous familial hypercholesterolemia.
Dosage
Take ezetimibe and simvastatin orally once daily in the evening with or without food. Maximum recommended dosage is 10/40 mg daily; 10/80 mg restricted to chronic users without muscle toxicity.
Side Effects
Common adverse effects (2% or greater and greater than placebo) include headache (5.8%), increased ALT (3.7%), myalgia (3.6%), upper respiratory tract infection (3.6%), and diarrhea (2.8%).
Interactions
Concomitant use of strong CYP3A4 inhibitors with ezetimibe and simvastatin is contraindicated. Dose adjustments required for certain medications.
Warnings
Risk of myopathy and rhabdomyolysis, particularly with higher doses. Temporarily discontinue in patients experiencing acute serious conditions at high risk of renal failure. Contraindicated with strong CYP3A4 inhibitors, cyclosporine, danazol, or gemfibrozil. Also contraindicated in acute liver failure or decompensated cirrhosis.
