Fenofibrate capsules are a PPAR alpha agonist available in 43 mg and 130 mg strengths for oral administration to treat lipid disorders.
Uses
Indicated as adjunct to diet to reduce triglyceride levels in adults with severe hypertriglyceridemia (500 mg/dL or greater) and to reduce elevated LDL-C in primary hyperlipidemia when recommended LDL-C lowering therapy is not possible.
Dosage
Severe hypertriglyceridemia: 43-130 mg orally once daily. Primary hyperlipidemia: 130 mg once daily. Administer as single dose with or without food. Assess triglycerides at 4-8 weeks; discontinue if inadequate response after 2 months.
Side Effects
Common adverse reactions (greater than 2%): abnormal liver tests, increased AST/ALT, increased CPK, and rhinitis. Serious reactions: hepatotoxicity, myopathy, rhabdomyolysis, cholelithiasis, and hypersensitivity reactions.
Interactions
Increased myopathy risk with statins and colchicine. Potentiates coumarin anticoagulants - monitor PT/INR. Immunosuppressants increase nephrotoxicity risk. Administer at least 1 hour before or 4-6 hours after bile acid resins.
Warnings
Serious drug-induced liver injury, including liver transplantation and death, has been reported. Monitor liver function at baseline and periodically. Risk of myopathy increased with statins or renal impairment. Contraindicated with severe renal impairment, active liver disease, pre-existing gallbladder disease.
Pregnancy
Limited available data insufficient to determine drug associated risk. Do not breastfeed during treatment and for 5 days after final dose.
