Fluconazole is a synthetic triazole antifungal available as oral tablets containing 50-200 mg for treatment of systemic fungal infections.

Uses

Indicated for treating vaginal candidiasis, oropharyngeal and esophageal candidiasis, and cryptococcal meningitis. Also used prophylactically to reduce candidiasis incidence in bone marrow transplant recipients receiving cytotoxic chemotherapy or radiation.

Interactions

Fluconazole is a moderate CYP2C9/CYP3A4 inhibitor and strong CYP2C19 inhibitor. Significantly increases cyclosporine levels (92% AUC increase), increases midazolam exposure 259%, increases phenytoin AUC 88%, increases warfarin prothrombin time. Coadministration with erythromycin, pimozide, and quinidine is contraindicated.

Warnings

Should be administered with caution to patients with liver dysfunction due to rare serious hepatic toxicity. Anaphylaxis reported in rare cases. Exfoliative skin disorders reported. Potential fetal harm with high-dose exposure during first trimester. Patients with hypokalemia and advanced cardiac failure are at increased risk for torsade de pointes.

Pregnancy

No adequate and well-controlled clinical trials in pregnant women. Case reports describe congenital anomalies with high-dose exposure (400-800 mg/day) in first trimester. Effective contraception recommended during high-dose treatment.
