Uses

Treatment of Major Depressive Disorder, Obsessive Compulsive Disorder, Bulimia Nervosa, and Panic Disorder. In combination with olanzapine for acute depressive episodes associated with Bipolar I Disorder and treatment-resistant depression.

Dosage

Adults: 20 mg/day initial dose for MDD/OCD, 60 mg/day for Bulimia, 10 mg/day initial for Panic. Pediatric dosing varies by indication. Maximum dose typically 80 mg/day.

Side Effects

Common side effects include nausea (22%), insomnia (19%), nervousness (13%), and tremor (9%). Sexual dysfunction, weight loss, and allergic reactions reported. Serious reactions include serotonin syndrome and QT prolongation.

Interactions

Risk of serotonin syndrome with serotonergic agents (triptans, TCAs, tramadol, lithium, St. John's Wort). QT interval prolongation with certain antipsychotics and antibiotics. CYP2D6 inhibition affects TCA metabolism.
