Uses
Fluvoxamine maleate tablets are indicated for treatment of obsessions and compulsions in patients with obsessive-compulsive disorder (OCD), as defined in DSM-III-R or DSM-IV.
Dosage
Adults: Start 50 mg at bedtime, increase by 50 mg every 4-7 days as tolerated, max 300 mg/day. Children/adolescents (8-17 yrs): Start 25 mg at bedtime, increase by 25 mg every 4-7 days, max 200 mg/day (ages 8-11) or 300 mg/day (ages 12-17).
Side Effects
Common reactions include nausea, somnolence, insomnia, asthenia, nervousness, dyspepsia, abnormal ejaculation, sweating, anorexia, tremor, and vomiting. 22% discontinued due to adverse reactions in trials.
Interactions
Significant interactions with benzodiazepines, carbamazepine, tricyclic antidepressants, warfarin, theophylline, and beta-blockers. Fluvoxamine inhibits multiple CYP450 isoenzymes.
Warnings
Black box warning: Antidepressants increase risk of suicidal thinking and behavior in children, adolescents, and young adults. Not approved for pediatric use except OCD patients. Monitor for clinical worsening and suicide risk. Screen for bipolar disorder. Risk of serotonin syndrome with serotonergic agents. Do not co-administer with MAOIs, tizanidine, thioridazine, alosetron, or pimozide.
Pregnancy
Prolonged experience has not identified clear drug-associated birth defect risk. Third-trimester exposure associated with increased postpartum hemorrhage risk and neonatal adaptation symptoms. Fluvoxamine present in breast milk.
