Hydrocodone bitartrate and acetaminophen is a combination opioid analgesic and non-opiate analgesic indicated for management of pain severe enough to require an opioid analgesic when alternative treatments are inadequate.

Uses
Indicated for management of pain severe enough to require an opioid analgesic when alternative treatments are inadequate. Should be reserved for patients who have not tolerated or benefited from non-opioid alternatives.
Dosage
Dosage should be individualized based on severity of pain, patient response, and prior analgesic experience. Available in 5mg/325mg, 7.5mg/325mg, and 10mg/325mg tablet strengths. Do not exceed 4,000 mg acetaminophen daily.
Side Effects
Most frequently reported: light-headedness, dizziness, sedation, nausea, vomiting. Other reactions include drowsiness, lethargy, anxiety, constipation, urinary retention, skin reactions, thrombocytopenia, serotonin syndrome, and anaphylaxis.
Interactions
CYP3A4 inhibitors (macrolides, azoles, protease inhibitors) increase plasma concentration. CYP3A4 inducers (rifampin, carbamazepine, phenytoin) decrease concentration. Benzodiazepines and CNS depressants increase overdose risk. Serotonergic drugs may cause serotonin syndrome. MAOIs contraindicated.

Black box warning: Serious and life-threatening risks including addiction, abuse, misuse, respiratory depression, accidental ingestion dangers, benzodiazepine interactions, neonatal withdrawal, and hepatotoxicity. Acetaminophen linked to acute liver failure at doses exceeding 4,000 mg daily. Contraindicated in significant respiratory depression, acute or severe bronchial asthma, GI obstruction.
Pregnancy
No adequate well-controlled studies in pregnant women. Extended use may cause neonatal opioid withdrawal syndrome. Not recommended during labor. Opioids cross placenta and may cause respiratory depression in neonates.
Storage
Store securely, out of sight and reach of children. Dispose of expired or unused medication by flushing if take-back option unavailable.
