Uses
Diminishes toxicity and counteracts effects of impaired methotrexate elimination and inadvertent overdosages of folic acid antagonists.
Dosage
Oral tablets up to 25 mg; doses exceeding 25 mg must be given parenterally. Initial leucovorin rescue: 15 mg every 6 hours until serum methotrexate levels fall below threshold.
Side Effects
Allergic sensitization including anaphylactoid reactions and urticaria reported with oral and parenteral administration.
Interactions
Large folic acid amounts may counteract antiepileptic effects of phenobarbital, phenytoin, and primidone. High-dose leucovorin may reduce intrathecal methotrexate efficacy.
Warnings
Administer as promptly as possible after overdosage. May enhance fluorouracil toxicity with reports of severe enterocolitis and deaths in elderly patients. Improper therapy for pernicious anemia and megaloblastic anemias from vitamin B12 deficiency.
Pregnancy
Animal studies not conducted. Give to pregnant women only if clearly needed.
