Uses

Indicated for use by females of reproductive potential to prevent pregnancy. Not recommended as long-term (>2 years) birth control unless other options are inadequate.

Dosage

150 mg via deep intramuscular injection every 3 months (13 weeks) in gluteal or deltoid muscle. First injection during first 5 days of menstrual period or within 5 days postpartum.

Side Effects

Common: menstrual irregularities (57% at 12 months), weight gain >10 lb (38% at 24 months), headache (16.5%), abdominal pain (11.2%), nervousness (10.8%).

Interactions

Enzyme inducers (barbiturates, carbamazepine, rifampin, St. John's wort) may decrease effectiveness. HIV protease inhibitors and antibiotics may affect progestin levels.

Warnings

Black box warning: Women who use MPA may lose significant bone mineral density. Bone loss greater with longer use, may not be completely reversible. Not recommended as long-term birth control (>2 years) unless other options inadequate. Risks include thromboembolic disorders, cancer, meningioma, ectopic pregnancy, anaphylaxis, liver function disturbances.

Pregnancy

No use in pregnancy; should discontinue if pregnancy occurs. No increased birth defect risk from prior exposure based on epidemiologic studies. Detectable in breast milk but appears safe. For exclusive breastfeeding, initiate at 6+ weeks postpartum.
