Naproxen and esomeprazole magnesium delayed-release tablets combine an NSAID with a proton pump inhibitor for arthritis symptom relief while reducing naproxen-associated gastric ulcer risk.

Uses

Indicated for adults and adolescents 12+ years requiring naproxen for arthritis relief (osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, juvenile idiopathic arthritis) with esomeprazole to decrease naproxen-associated gastric ulcer risk.

Dosage

Swallow tablets whole 30+ minutes before meals. Adults/adolescents >50 kg: one tablet twice daily of 375mg/20mg or 500mg/20mg. Adolescents 38-50 kg: one 375mg/20mg tablet twice daily. Avoid in moderate/severe renal or severe hepatic impairment.

Side Effects

Most common (>5%): gastritis and diarrhea. Others include upper respiratory infection, flatulence, headache, constipation, abdominal pain, and dizziness. Serious reactions possible including GI bleeding, cardiovascular events, and liver injury.

Interactions

Major interactions with anticoagulants (increased bleeding), clopidogrel (avoid), aspirin, ACE inhibitors/ARBs, diuretics, lithium, methotrexate, digoxin, and CYP2C19 inducers (St. John's Wort, rifampin - avoid).

Warnings

Black box warning: Risk of serious cardiovascular thrombotic events (MI, stroke) and GI bleeding/ulceration/perforation. Elderly and those with prior peptic ulcer disease or GI bleeding face greatest GI risk. Use lowest naproxen dose for shortest duration. Contraindicated post-CABG surgery and with rilpivirine-containing products.

Pregnancy

Avoid NSAIDs at approximately 30 weeks gestation onward due to premature ductus arteriosus closure risk. Limit use 20-30 weeks to lowest dose/shortest duration due to oligohydramnios/fetal renal dysfunction risk.
