Uses
Indicated to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure and reduced ejection fraction. Also indicated for pediatric heart failure in patients aged one year and older.
Dosage
Adult starting dose is 49 mg/51 mg orally twice daily with target maintenance of 97 mg/103 mg orally twice daily. Pediatric dosing is weight-based, ranging from 1.6 to 3.1 mg/kg twice daily.
Side Effects
Common adverse reactions include hypotension (18%), hyperkalemia (12%), cough (9%), dizziness (6%), and renal failure (5%).
Interactions
Avoid concomitant use with ACE inhibitors (36-hour washout required), aliskiren in renal impairment, potassium-sparing diuretics, NSAIDs, and lithium.
Warnings
Fetal toxicity: when pregnancy is detected, discontinue as soon as possible. Risk of angioedema, hypotension, impaired renal function, and hyperkalemia. Contraindicated with ACE inhibitors, history of angioedema, and aliskiren in diabetic patients.
