Ethacrynic acid is an unsaturated ketone derivative of aryloxyacetic acid, a potent loop diuretic available as white crystalline powder tablets.

Uses

Treatment of edema in congestive heart failure, cirrhosis, renal disease, nephrotic syndrome, ascites, and hospitalized pediatric patients with congenital heart disease.

Dosage

Adults: 50-100 mg initial dose; maintenance 50-200 mg daily in 25-50 mg increments. Pediatric: 25 mg initial with 25 mg stepwise increases.

Side Effects

Gastrointestinal effects, hyperuricemia, acute gout, deafness, tinnitus, vertigo, skin rash, headache, and thrombocytopenia reported.

Interactions

Avoid with lithium, aminoglycosides, cephalosporins; warfarin dosage may need reduction; NSAIDs may reduce diuretic effects.

Warnings

Risk of profound electrolyte and water loss; monitor weight and electrolytes; avoid rapid weight loss in elderly cardiac patients; deafness possible. Contraindicated in anuria, severe watery diarrhea history, and infants.

Pregnancy

Pregnancy Category B. No adequate and well-controlled studies in pregnant women but animal studies showed no fetal abnormalities.
